FINANCIAL IMPACT

Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Mammoth Lakes Fire Protection District (MLFPD) will be responsible for applicable annual fees which include medical oversight and emergency medical services vehicle inspections, currently $2,500 per agency and $500 per vehicle respectively, are outlined in Inland Counties Emergency Medical Agency’s (ICEMA) approved by the Board of Directors on March 24, 2026 (Item No. 73). Sufficient appropriation and revenue have been included in ICEMA’s budget and will be included in future recommended budgets.

BACKGROUND INFORMATION

ICEMA, as the local Emergency Medical Services (EMS) agency, is responsible for planning, implementing, and evaluating the EMS system within San Bernardino, Inyo, and Mono Counties. Pursuant to California Health and Safety Code section 1797.218, ICEMA is authorized to approve Basic Life Support (BLS) and Advanced Life Support (ALS) programs for the delivery of emergency medical care.

ICEMA is responsible for establishing EMS protocols, policies, and procedures within its jurisdiction and for the certification and accreditation of EMS personnel. MLFPD provides BLS service to residents and visitors within its service area in Mono County and desires to provide ALS transport services in accordance with ICEMA standards and protocols. Mono County has identified a community and operational need for MLFPD to provide additional EMS resources. Mono County currently provides ALS ambulance transport services, and MLFPD’s ALS transport services will supplement existing EMS resources when needed and resources are available.

The proposed Agreement authorizes MLFPD to provide BLS and ALS transport services within the MLFPD Sphere of Influence geographic area identified in Exhibit 1 of the Authorization Agreement (Agreement). Under the Agreement, MLFPD will continue to provide BLS transport services 24 hours per day, seven days per week, and may provide ALS transport services when reasonably necessary and appropriate based upon EMS system needs and available resources. MLFPD may also provide mutual aid response when requested to contiguous emergency medical response areas.

When operating as an ALS unit, MLFPD is required to staff the unit with at least one ICEMA-accredited paramedic and one State-certified Emergency Medical Technician. MLFPD is also required to comply with ICEMA protocols, policies, procedures, training requirements, electronic patient care reporting and data collection requirements, and applicable State regulations governing emergency medical care.

The Agreement will commence on November 3, 2026, provided it has been executed by both parties, and will continue through November 2, 2031. Either party may terminate the Agreement upon 90 days' written notice.

Approval of the proposed Agreement will authorize MLFPD to provide BLS and ALS transport services within the identified service area under ICEMA medical oversight and in accordance with applicable ICEMA standards, policies, procedures, and protocols.
