RECOMMENDATION(S)

Recommendation

1.                     Approve Revenue Agreement (State Revenue Agreement No. AD-2627-12) with the California Department of Aging to provide Aging and Disability Resource Connection program services, in the amount of $621,650, for the period of July 1, 2026 through June 30, 2027.

2.                     Authorize the Chair of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Aging and Adult Services, to execute all non-substantive amendments in relation to the Revenue Agreement, on behalf of the County, subject to review by County Counsel.

3.                     Direct the Director of the Department of Aging and Adult Services to transmit all non-substantive amendments in relation to the Revenue Agreement to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT

Approval of this item does not require additional Discretionary General Funding (Net County Cost).  The Revenue Agreement from the California Department of Aging (CDA) for the Aging and Disability Resource Connection (ADRC) program in the amount of $621,650 is 100% state funded.  Adequate appropriation and revenue have been included in the Department of Aging and Adult Services’ (DAAS) recommended 2026-27 budget.

 

BACKGROUND INFORMATION

California Senate Bill 80 established the first state ADRC Infrastructure Grants Program (Grant Program) in 2019.  The Grant Program allows CDA to award grants that support eligible Emerging and Designated ADRC partnerships in developing, implementing, and sustaining a local No Wrong Door (NWD) System.  California’s ADRCs are led by a core partnership between Independent Living Centers (ILC) and Area Agencies on Aging (AAA).  They also include an extended network of partners, such as Molina Healthcare, Inland Empire Health Plan, Inland Regional Center, Inland Southern California 211+ that may provide various ADRC services.

 

San Bernardino County (County) established its ADRC in November 2019 through a collaborative partnership between the County’s designated ILC, Rolling Start Inc., and DAAS, the County’s local AAA.  The purpose of the ADRC program is to develop a coordinated network of programs and services under the NWD System to serve the needs of older adults, people with disabilities, and caregivers.  The ADRC helps individuals navigate the fragmented and complicated system of Long-Term Services and Supports (LTSS) and achieve their personal goals and preferences for healthy aging.  The ADRC assists in enabling consumers to access all LTSS programs through one agency, organization, coordinated network, or portal.

 

The ADRC provides comprehensive information and referrals, offering warm hand-offs and follow-up to ensure individuals connect with the right program and/or services.  ADRC core services and functions include: enhancing information and referral services through searchable resource systems; providing options counseling to support the person-centered planning on needed services and available programs; short-term coordination for the purpose of stabilizing a situation in which a person may be at risk; and transitional services supporting the coordination of transitioning from health care facilities to home.

 

Approval of this item will allow DAAS to accept the revenue for these services for budget year 2026-27.  The funding supports the development, implementation, and long-term sustainability of the NWD System to enable people of all ages, income, and disabilities to connect with any one ADRC partner organization for accessing a wide array of LTSS.  These resources strengthen the infrastructure needed to provide coordinated access to services and are estimated to assist approximately 4,200 consumers during the funding year.  The ADRC reduces confusion, enhances individual choice, and supports informed decision-making for those seeking LTSS.
