Recommendation
Accept and approve Amendment No. 1 to Agreement No. 25-03 (State Grant Agreement No. 24-10568), from the California Department of Public Health for the California Ending the Epidemics STI*HIV*HEP C Grant Program, formerly referred to as High-Impact HIV Prevention and Surveillance Programs for Health Departments, decreasing the total amount of the agreement by $17,412, from $4,492,021 to $4,474,609, with no change to the agreement period of August 1, 2024 through May 31, 2029.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Amendment No. 1 to Agreement No. 25-03, in the amount of $4,474,609, funded by the California Department of Public Health’s (CDPH) California Ending the Epidemics STI*HIV*HEP C Grant Program (EHE), does not require a local match. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2026-27 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
CDPH provides grant funding to local health jurisdictions to support efforts to end the human immunodeficiency virus (HIV) epidemic in America. CDPH collaborates with these jurisdictions to collect, analyze, and disseminate surveillance data on people living with or diagnosed with HIV in California. EHE funding helps DPH meet state goals by supporting comprehensive approaches to HIV prevention and care, dismantling structural barriers to services, ensure health services are accessible to everyone, and improve the overall health of people with HIV and those who can benefit from prevention services.
DPH will use the EHE funds to implement various activities aimed to decrease HIV diagnoses across the county. These efforts include expanding routine screening in healthcare settings such as the Easy Access Clinic, Mobile Testing Units, and via co-locating DPH staff at Arrowhead Regional Medical Center. DPH will also focus on expanding local testing programs to reach individuals in non-healthcare settings. This funding will support collaborative practice and capacity-building among community partners and further expand harm reduction services for substance users at risk for HIV infection. In 2025-26, this grant supported HIV prevention services to 992 unduplicated individuals, plus an additional 357 Pre-Exposure Prophylaxis-focused outreach encounters. Additionally, DPH will use EHE funding to deliver linkage-to-care services and promote timely initiation of antiretroviral therapy, support re-engagement and retention in care and treatment, increase use of Pre-Exposure Prophylaxis services, support outbreak response and investigation of HIV clusters, and expand the use of integrated approaches that address interconnected comprehensive care related to HIV, Hepatitis C, and Sexually Transmitted Infections.
On January 14, 2025 (Item No. 45), the Board of Supervisors (Board) approved grant award Agreement No. 25-03 (State Grant Agreement No. 24-10568), including non-standard terms, from CDPH for EHE funding, in the amount of $4,492,021, retroactively for a five-year period of August 1, 2024 through May 31, 2029.
On June 25, 2026, DPH received Amendment No. 1 to Agreement No. 25-03 from CDPH, decreasing EHE funding by $17,412, for a total amount of $4,474,609, allocating $771,921 in the first year, and annual allocations of up to $925,672 for the remainder of the grant period. CDPH will issue Letters of Award for each subsequent year throughout the grant period.
The grant award provides annual non-competitive formula funding to support the EHE activities from June 1, 2026 through May 31, 2027, contingent upon available appropriations and federal award. Based on this formula, CDPH has decreased funding by a total amount of $17,412 for years two through five of the grant performance period. This decrease of $4,353 per year will not impact DPH’s HIV prevention services.
This item is being presented to the Board at this time, as this is the first date available following the required operational, fiscal, and legal reviews. Approval by the Board at this time will have no impact on DPH’s services or activities. This grant award provides reimbursement to DPH for EHE services performed between August 1, 2024 through May 31, 2029.
