Recommendation
1. Ratify the emergency purchase of an unbudgeted fixed asset, an ultrasound machine, from FUJIFILM Sonosite, Inc. in the amount of $54,931.
2. Authorize the Auditor-Controller/Treasurer/Tax Collector to post the $54,931 as a budget adjustment for 2021-22, as identified in the Financial Impact Section (Four votes required).

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General funding (Net County Cost). The $50,980 purchase cost plus estimated $3,951 in taxes for a total cost of $54,931 will be funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative actively related to the repeal and/or replacement of the Affordable Care Act. This item authorizes the Auditor-Controller/Tax Collector to post the following adjustments to the ARMC 2021-22 budget.

BACKGROUND INFORMATION
On Thursday, June 2, 2022, Arrowhead Regional Medical Center (ARMC) made an emergency purchase of an unbudgeted, fixed asset for the ARMC Oncology Department. The capital item is a FUJIFILM Sonosite PX Ultrasound System that is used for diagnostic treatment of breast cancer patients that receive care at ARMC. The existing ultrasound equipment was not functioning properly and was, therefore, impacting patient care. Furthermore, the ARMC Oncology Department was informed that the manufacturer would no longer perform repair work on the machine due to its age. Therefore, the purchase could not wait until the new fiscal year, and an immediate purchase was needed to ensure continuity of care for the cancer patients receiving treatment at the hospital.
The purchase was made under the Vizient Supplier Agreement between Vizient and FUJIFILM. ARMC recommends the approval of this unbudgeted fixed asset to ensure quality patient care is maintained in the ARMC Oncology Department. Per County Policy 11-04, the purchase of unbudgeted fixed assets of $10,000 per unit value or more requires Board approval. The policy also requires Board approval where emergency purchases of fixed assets exceeding $10,000 are made for the preservation of life. Given that this equipment is necessary for the diagnostic treatment of breast cancer patients that receive care at ARMC, the hospital proceeded with the purchase, with County Counsel and Purchasing approval, in accordance with County policy.
PROCUREMENT
The County Purchasing Department approved the competitive procurement of one FUJIFILM Sonosite PX Ultrasound System based on Vizient agreement XR0433.
