Body

COUNTY AND CHIEF EXECUTIVE OFFICER GOALS & OBJECTIVES

Provide for the Safety, Health and Social Service Needs of County Residents.

FINANCIAL IMPACT

Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The cost of $300,000 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2026-27 budget and will be included in future recommended budgets.

BACKGROUND INFORMATION

Approval of this Amendment will allow for the continued use of coagulation analyzers to assist clinicians in the diagnosis and treatment of ARMC patients. The coagulation analyzers are utilized for routine and specialized patient coagulation (blood clotting) testing that provides clinicians with critical information to monitor a patient's ability to form blood clots. This information is used in screening for surgical procedures, diagnosis of bleeding and clotting disorders, and monitoring of anticoagulant medications. Additionally, extension of the agreement avoids redundant interface costs that would be incurred if a new analyzer were purchased.

Diagnostica Stago offers coagulation testing systems featuring both mechanical and optical clot detection technologies. This flexibility enables ARMC to select an appropriate detection method for specific assays and sample characteristics. Mechanical detection can help minimize the impact of optical interference from hemolysis, icterus, and lipemia, supporting reliable coagulation testing when used in accordance with assay-specific performance characteristics. Diagnostica Stago differentiates itself primarily through deep specialization in hemostasis and thrombosis, an integrated analyzer-and-reagent portfolio, and a long-standing focus on coagulation science.

The $300,000 increase to the contract amount will accommodate anticipated patient coagulation testing needs during the one-year extension, including estimated testing volume and utilization and associated reagent and service costs through 2028. The increase will ensure continuity of patient coagulation testing services through the extended contract term. The increased spend in this Amendment exceeds the authority for approval by the Purchasing Department.

On August 18, 2022, the Purchasing Department approved an agreement with Diagnostica Stago, Inc. (SAP Contract No. 4400020301) for patient coagulation testing, in the amount of $539,942.40 for the total period of December 1, 2022 through November 30, 2027. The Purchasing Department approved this procurement based on functional specifications, as Diagnostica Stago offers a unique coagulation testing portfolio that includes both mechanical and optical clot detection technologies, which are currently established and in use at ARMC’s Laboratory.

ARMC recommends approval of the amendment with Diagnostica Stago, Inc., as it will enable ARMC to perform essential testing for San Bernardino County residents receiving treatment at the hospital.
