Amendments to Contracts for HIV Care Program and Minority AIDS Initiative Services under the Ryan White Part B Program

RECOMMENDATION(S)
Recommendation
Approve Amendment No. 4 to contracts with the following agencies to provide services under the Ryan White Part B Program, decreasing the total aggregate amount of the contracts by $268,162, from $3,512,748 to $3,244,586, with no change to the contract terms of April 1, 2021 through March 31, 2026:
1. Desert AIDS Project dba DAP Health, Contract No. 21-09, increasing the contract amount by $67,386, from $252,174 to $319,560.
2. Foothill AIDS Project, Contract No. 21-10, decreasing the contract amount by $246,100, from $2,569,195 to $2,323,095.
3. Young Scholars for Academic Empowerment dba TruEvolution, Inc., Contract No. 21-11, decreasing the contract amount by $89,448 from $691,379 to $601,931.

FINANCIAL IMPACT
Approval of this item will not require the use of Discretionary General Funding (Net County Cost). The amendments decrease the contract amounts by an aggregate total of $268,162. These services are funded by the Ryan White Part B HIV Care Program (Care Program) and Part B Minority AIDS Initiative (Initiative) through the California Department of Public Health (CDPH). Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2023-24 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
CDPH allocates grants to local health jurisdictions to administer the Ryan White Part B Program, which includes the Care Program and Initiative. The Care Program provides services to low-income people affected by HIV and AIDS. The Initiative supports improvements in HIV-related health outcomes for racial/ethnic minority communities and reductions of HIV-related health disparities. Beginning April 1, 2024, CDPH reduced the overall DPH allocation as they are only funding the Care Program and are no longer funding the Initiative.
On January 5, 2021 (Item No. 24), as a result of a formal procurement, the Board of Supervisors (Board) approved Contract No. 21-09 with Desert AIDS Project dba DAP Health, Contract No. 21-10 with Foothill AIDS Project, and Contract No. 21-11 with Young Scholars for Academic Empowerment dba TruEvolution (collectively known as Contracts), in the aggregate amount of $2,245,530, for the term of April 1, 2021 through March 31, 2024.
On March 23, 2021 (Item No. 51), the Board approved Amendment No. 1 to the Contracts, decreasing the total aggregate amount by $111,690, from $2,245,530 to $2,133,840, with no change to the term of April 1, 2021 through March 31, 2024, due to a decrease in the Care Program and Initiative allocation to DPH.
On November 14, 2023 (Item No. 59), the Board approved Amendment No. 2 to the Contracts, increasing the total aggregate amount by $1,280,273, from $2,133,840 to $3,414,113, and extending the contract term by two years, for a total term of April 1, 2021 through March 31, 2026, due to an increase in the Care Program and Initiative allocation to DPH.
On March 26, 2024 (Item No. 56), the Board approved Amendment No. 3 to the Contracts, increasing the total aggregate amount by $98,635, from $3,414,113 to $3,512,748, with no change to the contract term of April 1, 2021 through March 31, 2026, due to an increase in the Care Program and Initiative allocation to DPH.
Approval of the Recommendations will decrease the aggregate amount of the Contracts by $268,162, due to DPH’s reduced allocation from the state. DPH reviewed the populations being served and determined Desert AIDS Project dba DAP Health’s allocation required a slight increase due to the current volume of patients they are serving. However, the remaining funding is split between the two other contractors and utilized internally by DPH. This resulted in reduced allocations to two of the Care Program contractors and DPH internal funding. Services to patients in communities of color, which include African Americans, Latinos, and American Indians under the Initiative are not impacted, as funding is utilized from other Ryan White sources, including the Care Program, to ensure HIV-related health disparities continue to be addressed. This includes continuing targeted outreach to communities of color to reduce disparities in care. Reducing disparities in care and improving health outcomes for persons living with HIV are also requirements under the Care Program.
Contractor performance is measured by review of monthly reporting and required data submission, and an annual site visit to monitor compliance with the administrative, program, and fiscal terms of the Contracts.
