Recommendation
1. Approve Contract with the City of Rancho Cucamonga, to contribute funding from the 2025-26 Second District’s Board of Supervisors Discretionary Fund - District Specific Priorities Program budget to assist with the Rancho Cucamonga Family Resource Center/Neighborhood Center Improvement project at 9791 Arrow Route in Rancho Cucamonga, in the not-to-exceed amount of $900,000, for the period of June 9, 2026 through June 8, 2028.
2. Approve Amendment No. 1 to Contract No. 25-917 with Andrew Manganaro, M.D, PC (d/b/a Life Line Mobile Screening), for voucher-based wellness and prevention screening program, increasing the not-to-exceed contract amount by $100,000, from $398,000 to the total not-to-exceed amount of $498,000, from the 2025-26 Second District’s Board of Supervisors Discretionary Fund - District Specific Priorities Program budget, revising the scope of services to add Alzheimer’s Disease Risk Screening Testing, and extending the term by 12 months, for a total term of November 18, 2025 through December 31, 2028, and waiving the requirement in County Policy 05-10 for a financial matching contribution of at least 25% from Andrew Manganaro, M.D, PC.
3. Approve Contract with The West End Young Men’s Christian Association, to contribute funding from the 2025-26 Second District’s Board of Supervisors Discretionary Fund - District Specific Priorities Program budget to provide two-year support to its Safety Around Water program, in the not-to-exceed amount of $60,000, for the period of June 9, 2026 through June 8, 2028.
4. Approve Contract with the Fontana Chamber of Commerce, to contribute funding from the 2025-26 Second District’s Board of Supervisors Discretionary Fund - District Specific Priorities Program budget to assist with program and event costs, including the implementation of a new workforce system designed to improve access to employment resources and streamline connections between job seekers and employers, in the not-to-exceed amount of $25,000, for the period of June 9, 2026 through June 8, 2027.
5. Approve Contract with the Rise to Grind Foundation, to contribute funding from the 2025-26 Second District’s Board of Supervisors Discretionary Fund - District Specific Priorities Program budget to support the 2026 Back to School event at the Red Hill Community Park located at 7484 Vineyard Avenue in Rancho Cucamonga, on or about July 25, 2026, in the not-to-exceed amount of $10,000, for the period of June 9, 2026 through June 8, 2027.

FINANCIAL IMPACT
Approval of this item will not result in the use of additional Discretionary General Funding (Net County Cost). On September 21, 2021 (Item No. 18), November 16, 2021 (Item No. 33), February 6, 2024 (Item No. 61), June 11, 2024 (Item No. 110), April 29, 2025 (Item No. 86), and June 10, 2025 (Item No. 136), the Board of Supervisors (Board) approved an allocation of $4 million, $7 million, $6 million, $3 million, $2.5 million, and $2.5 million, respectively, to the Board Discretionary Fund - District Specific Priorities Program (Priorities Program) for each of the five supervisorial districts for projects that meet a public purpose, and provide services to citizens that promote health, safety, economic well-being, and other public services that enhance quality of life and meet the needs of County residents. On an annual basis, unspent appropriations for discretionary funding will be carried over into the subsequent year’s budget.
The recommended projects will be funded by the Second District’s Priorities Program budget.
BACKGROUND INFORMATION
Priorities Program projects help promote the vision of a complete County by recognizing the unique traits of each Supervisorial District and, thereby, allow individual Board Districts to work with community partners through County services and contractual agreements to identify programs, projects, and initiatives that support the vision, meet a public purpose and provide services to residents that promote health, safety, economic well-being, education, recreation, and other public services that enhance quality of life and meet the needs of the County’s residents. Projects under the Priorities Program are based on District Supervisor discretion and are presented by each District Supervisor for Board approval.
Recommendation No. 1 will assist the City of Rancho Cucamonga (Rancho) to fund the improvement project at the Rancho Cucamonga Family Resource Center/Neighborhood Center (Center) located at 9791 Arrow Route in Rancho Cucamonga. The Center improvements include, but are not limited to, roof replacement, exterior paint and signage, restroom upgrades, landscaping, parking lot paving, and the purchase of furniture and equipment. This contract serves the public purpose of providing for the health, safety, recreational, and social service needs of County residents through the improvement and revitalization of the Center to provide a community hub delivering social services, recreation programs, and special events while reducing long-term maintenance costs and improving operational efficiency. Rancho acknowledges and agrees that it will make a matching contribution of at least 25% of the total cost of the project per County Policy 05-10.
On November 18, 2025 (Item No. 30), the Board approved Contract No. 25-917 with Andrew Manganaro, M.D., PC (d/b/a Life Line Mobile Screening) (LLMS) in the not-to-exceed amount of $398,000, to provide a voucher-based wellness and prevention screening program to County residents and first responders for the period of November 18, 2025 through December 31, 2027. The County Purchasing Department approved the non-competitive procurement of LLMS based on its specialized credentials.
As reflected under Recommendation No. 2, Amendment No. 1 to Contract No. 25-917 is requested to add Alzheimer’s Disease Risk Screening Tests for County residents 55 years of age and older to support early screening and potential detection of Alzheimer’s disease through blood-based biomarker screening that measures p-tau217 protein levels associated with changes linked to Alzheimer’s Disease. Approval of Recommendation No. 2 will increase the contract amount by $100,000, from $398,000 to a total not-to-exceed amount of $498,000, revise the scope of work to include 250 voucher-based Alzheimer’s Disease Risk Screening Tests for eligible County residents, and extend the contract term by 12 months, for a revised contract period of November 18, 2025 through December 31, 2028. This amendment will continue to serve the public purpose of supporting the health, public safety, and social service needs of County residents by providing one-time vouchers for participation in health screening events conducted by LLMS.
It’s also requested that the Board waive the minimum 25% matching contribution as the funding will allow the County to immediately implement preventive health screening opportunities and reduce transportation and scheduling barriers that often prevent residents from seeking early detection services. On November 18, 2025 (Item No. 30), the Board previously waived the matching contribution for this reason.
Recommendation No. 3 will assist The West End Young Men’s Christian Association (YMCA) to provide two-year support to their Safety Around Water program which is a year-round initiative dedicated to teaching youth essential water safety and basic life-saving skills. This contract serves the purpose of providing for the health, safety, recreational and social service needs of the County residents through the YMCA’s Safety Around Water program to help provide water safety instruction and resources to local youth and families, strengthen partnerships with schools and community organizations, and promote drowning prevention and public safety awareness throughout the region. YMCA is a non-profit organization. Per County Policy 05-10, non-profit organizations are not required to provide a minimum 25% matching contribution to the projects.
Recommendation No. 4 will assist the Fontana Chamber of Commerce (FCC) with costs related to programs and events offered by FCC. This contract serves the public purpose of providing for the health, economic and social service needs of the County residents through the implementation of a new workforce system and the expansion of community and business programs, including S.E.R.V.E. (Support, Empower, Resource, Veteran, and Engage) Veterans, Boss Babes Network, and Morning Mentoring, to provide mentorship, networking, educational, and professional development opportunities to veterans, entrepreneurs, small businesses, and other members of the community. FCC is a non-profit organization. Per County Policy 05-10, non-profit organizations are not required to provide a minimum 25% matching contribution to the projects.
Recommendation No. 5 will assist the Rise to Grind Foundation (RGF) with costs related to the 2026 Back to School event on or about July 25, 2026, hosted by RGF at the Red Hill Community Park located at 7484 Vineyard Avenue in Rancho Cucamonga. This contract serves the public purpose of providing for the educational and social service needs of the County residents through the 2026 Back to School event to help students in the Second District prepare for the academic year and promote youth empowerment, family support, and long-term educational success in the communities. RGF is a non-profit organization. Per County Policy 05-10, non-profit organizations are not required to provide a minimum 25% matching contribution to the projects.
Under the Priorities Program contract terms, the Chief Executive Officer (CEO), at the direction of the Second District Supervisor, may extend the terms of the contracts, in writing, under the following conditions:
a. In aggregate, all extensions do not exceed 12 calendar months;
b. Is specifically requested by the contractor;
c. Will not change the project goals or scope of services;
d. Is in the best interest of the County and contractor in performing the scope of services under the contract; and
e. Does not alter the amount of compensation under the contract.
Board discretionary funding allocations under the Priorities Program with community partners must be for projects or services of $10,000 or more as described in County Policy 05-10. Additionally, contracts that include non-profit organizations, government entities, or educational institutions are non-competitive. Contracts with community partners that include for-profit organizations are to be processed according to existing County procurement policies. Contracts with other government agencies or for-profit organizations require a matching contribution of at least 25% of the proposed projects. All contracts shall be paid on a lump-sum basis or under other terms as approved by the Board.
