RECOMMENDATION(S)

Recommendation

Accept and approve Amendment No. 1 to Contract No. 26-168 (Award No. 6 H80CS00657-25-01) from the United States Department of Health and Human Services, Health Resources and Services Administration, accepting an amended grant award for continued operational support of the Federally Qualified Health Centers, increasing the amount by $1,375,920, from $1,245,920 to $2,621,840, with no change to the period of March 1, 2026 through February 28, 2027.

FINANCIAL IMPACT

Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The amended grant award  in the amount of $1,375,920, from the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA), does not require a local match. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2026-27 budget.

 

BACKGROUND INFORMATION

HRSA provides a three-year Service Area Competition (SAC) grant awards under the Health Center Program to support a variety of health-related services provided at DPH’s Federally Qualified Health Centers (FQHC) located in Adelanto, Hesperia, Ontario, and San Bernardino, and two school-based sites located in Apple Valley and Ontario. DPH has received SAC funding since 2017.

 

On August 8, 2023 (Item No. 35), and September 12, 2023 (Item No. 42), the Board of Supervisors (Board) approved Phase 1 and Phase 2, respectively, of the HRSA SAC grant application, in the amount of $2,291,840 annually, for the period March 1, 2024 through February 28, 2027.

 

On March 12, 2024 (Item No. 30), the Board approved Contract No. 24-210 (Award No. 2 H80CS00657-23-00), in the amount of $954,933 and Contract No. 24-211 (Award No. 6 H80CS00657-23-01), in the amount of $190,987, for the total amount of $1,145,920 in SAC funding, for the period of March 1, 2024 through February 28, 2025. The grant awards were erroneously filed as separate contracts. To resolve the discrepancy, subsequent amendments and associated grant amounts have been included under Contract No. 24-211.

 

On May 7, 2024 (Item No. 41), the Board approved Amendment No. 1 to Contract No. 24-211 (Award No. 6 H80CS00657-23-02), for an additional $100,000 in SAC funding, for a total amount of $1,245,920, for the period of March 1, 2024 through February 28, 2025.

 

On June 11, 2024 (Item No. 65) the Board approved Amendment No. 2 to Contract No. 24-211 (Award No. 6 H80CS00657-23-03) for an additional $1,145,920 in SAC funding, for a total amount of $2,391,840, for the period of March 1, 2024 through February 28, 2025.

 

On June 23, 2024 (Item No. 46), the Board approved Amendment No. 3 to Contract No. 24-211 (Award No. 6 H80CS00657-23-04), for an additional $35,260 in SAC funding, for a total amount of $2,427,100, for the period of March 1, 2024 through February 28, 2025.

 

Between August 14, 2024 and September 18, 2024, DPH received three non-financial notices (Award Nos. 6 H80CS00657-23-05, 6 H80CS00657-23-06 and 6 H80CS00657-23-09) from HRSA documenting administrative actions.

 

On November 5, 2024 (Item No. 35), the Board approved Amendment Nos. 4, 5, and 6 to Contract No 24-211 (Award Nos. 6 H80CS00657-23-07 for $66,667, 6 H80CS00657-23-08 for $28,500 and 6 H80CS00657-23-10 for $37,000), for an additional $132,167 in SAC funding, for a total amount of $2,559,267, for the period of March 1, 2024 through February 28, 2025.

 

On February 11, 2025 (Item No. 31), the Board approved Contract No. 25-102 (Award No. 5 H80CS00657-24-00), for SAC funding in the amount of $1,038,267, for the period of March 1, 2025 through February 28, 2026.

 

On April 29, 2025 (Item No. 51), the Board approved Amendment No. 1 to Contract No. 25-102 (Award No. 6 H80CS00657-24-02), for an additional $207,653 in SAC funding, for a total of $1,245,920, for the period of March 1, 2025 through February 28, 2026.

 

On September 9, 2025 (Item No. 48) the Board approved Amendment No. 2 to Contract No. 25-102 (Award No. 6 H80CS00657-24-04), for an additional $1,245,920 in SAC funding for a total amount of $2,491,840, for the period of March 1, 2025 through February 28, 2026.

 

On March 10, 2026 (Item No. 30) the Board approved Contract No. 26-168 (Award No. 5 H80CS00657-25-00), in the amount of $1,245,920, for the period of March 1, 2026 through February 28, 2027.

 

On May 21, 2026, DPH received notice of Amendment No. 1 to Contract No. 26-168 (Award No. 6 H80CS00657-25-01), for an additional $1,375,920 in SAC funding, for a total of $2,621,840, for the period of March 1, 2026 through February 28, 2027. DPH will return to the Board to accept further SAC funding from HRSA if and when it is received.

 

This item is being presented at this time, as this is the first available date following the required operational, fiscal, and legal reviews. Approval of this item will not impact DPH’s FQHC services or activities.
