Recommendation
1. Accept and approve Amendment No. 3 to Agreement No. 23-833 with the California Department of Public Health for the Future of Public Health funding, to provide continued support for public health workforce and infrastructure, increasing the amount by $11,015,929, from $33,319,926 to $44,335,855, and extending the performance period by one year, for the period of July 1, 2023 through June 30, 2027.
2. Approve and authorize the submission of the one-year Acknowledgement of Allocation Letter, Annual Certification, and Annual Spend Plan, as required by the California Department of Public Health, to continue to receive Future of Public Health funding through June 30, 2027.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The Department of Public Health’s (DPH) 2026-27 allocation from the California Department of Public Health (CDPH) for Future of Public Health (FoPH) funding is $11,015,929. The FoPH funding does not require a local match. Adequate appropriation and revenue have been included in the DPH 2026-27 budget.
BACKGROUND INFORMATION
CDPH provides $188,200,000 annually in FoPH funding to local health jurisdictions to support the public health workforce and infrastructure. CDPH requires that at least 70% of the funds support the permanent public health workforce, and authorizes up to 30% for equipment, supplies, and other administrative cost such as facility space, furnishings, and travel. DPH verifies compliance with the workforce and infrastructure requirements by tracking the use of FoPH funding through program codes in the Employee Management and Compensation System to fund positions within multiple DPH programs that historically would not have been grant- or fee-funded. The use of the remaining FoPH funding is tracked through the County’s SAP system. In addition, DPH is required to electronically submit an annual work plan and spend plan to CDPH for review and consideration of each fiscal year’s allocation.
On August 8, 2023 (Item No. 37), the Board of Supervisors (Board) approved Agreement No. 23-833 from CDPH for FoPH funding, with a total allocation of $33,853,248, for the period of July 1, 2023 through June 30, 2026, which reflected a $11,284,416 annual allocation between 2023-24 and 2025-26.
On September 24, 2024 (Item No. 58), the Board approved Amendment No. 1 to Agreement No. 23-833, with CDPH reducing the allocation of FoPH funding to support public health workforce and infrastructure by $529,670, from $33,853,248 to $33,323,578, for the period of July 1, 2023 through June 30, 2026, to reflect DPH’s portion of a statewide FoPH annual allocation reduction effective 2024-25.
On August 19, 2025 (Item No. 47), the Board approved Amendment No. 2 to Agreement No. 23-833, with CDPH for FoPH funding to provide continued support for public health infrastructure, decreasing the anticipated 2025-26 funding by $3,652, from $11,019,581 to $11,015,929, thereby decreasing the total agreement amount from $33,323,578 to $33,319,926, for the total grant performance period of July 1, 2023 through June 30, 2026.
On July 1, 2026, DPH received an allocation notice from CDPH to Agreement No. 23-833, increasing the agreement amount by $11,015,929, from $33,319,926 to $44,335,855, for the grant performance period of July 1, 2023 through June 30, 2027. DPH will return to the Board to accept further FoPH funding from CDPH, if and when it is received.
This item is being presented to the Board at this time as this is the first available date following the required operational, fiscal, and legal review. Approval at this time will not impact DPH services or activities.
Approval of this item will allow DPH to accept the FoPH funding allocation and submit the Acknowledgment of Allocation Letter, Annual Certification, and Annual Spend Plan to CDPH, as required to secure funding for the 2026-27 fiscal year.
