RECOMMENDATION(S)

Recommendation

1.                     Accept a grant award (State Award No. 1936BASE00/1936FSIE00) from the California Department of Public Health, Tuberculosis Control Branch, in the amount of $385,003, for continued support of tuberculosis prevention and control activities, for the grant period of July 1, 2019 through June 30, 2020.

2.                     Designate the Chairman of the Board of Supervisors, the Chief Executive Officer, or the Director of the Department of Public Health to execute any subsequent non-substantive grant award amendments in relation to the grant award (State Award No. 1936BASE00/1936FSIE00) on behalf of the County, subject to review by County Counsel.

3.                     Direct the Chairman of the Board of Supervisors, the Chief Executive Officer, or the Director of the Department of Public Health to transmit all documents and amendments in relation to this grant award to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost).  The California Department of Public Health (CDPH), Tuberculosis (TB) Control Branch does not require a match for this grant award.  The total grant award of $385,003 consists of a base award ($363,608) and food, shelter, incentives, and enablers (FSIE) allocation ($21,395) and is approximately 55% federal and 45% state funded.  Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2019-20 budget.

 

BACKGROUND INFORMATION

DPH continues to receive reports of suspected and confirmed cases of TB, a highly infectious disease that can cause serious illness, including death.  More than 3,400 investigations of suspected TB cases were conducted in calendar year 2018 resulting in 54 cases of active TB identified in San Bernardino County.  TB is second only to HIV/AIDS as the greatest killer worldwide due to a single infectious agent.  Inadequately treated TB can cause multi-drug resistant TB, which costs thousands of dollars and takes 18 to 24 months to cure.  DPH staff serve as resources and consultants to physicians and hospitals regarding the treatment and management of confirmed and suspected cases of TB and their contacts.

 

The grant award from the CDPH TB Control Branch supports DPH’s efforts to control the spread of TB countywide.  DPH has received this funding since 1948 and will use it to maintain TB control standards, investigate reported and suspected cases of TB, provide direct observed therapy, enhance treatment compliance, offer alternative housing for persons with infectious TB to increase successful follow-up treatment, complete state reporting requirements, and use less restrictive alternatives to decrease the need for detention of TB patients.

 

On April 30, 2019, the Chief Executive Officer approved the submission of a noncompetitive grant application to the CDPH TB Control Branch for TB base award funding, in the amount of $363,608, for the grant period of July 1, 2019 through June 30, 2020.  The FSIE funds in the amount of $21,395 are an allocation, which is not part of the grant application process, and included by CDPH as part of the total award for TB prevention and control activities.  The base award must be used exclusively for TB related activities.  The FSIE allocation must be used to provide services that include measures to enhance compliance, prevent homelessness, and allow for the use of less restrictive alternatives to decrease or prevent the need for detention. 

 

On June 26, 2019, DPH received the grant award from the CDPH TB Control Branch, in the amount of $385,003, for the grant period of July 1, 2019 through June 30, 2020.  In addition to the acceptance of the grant award to support TB prevention and control activities, DPH is requesting approval for the Chairman of the Board, the Chief Executive Officer, or the DPH Director to execute any subsequent non-substantive amendments to the grant award, on behalf of the County, subject to review by County Counsel. 

 

This item is being presented to the Board of Supervisors at this time, as this is the first date available for presentation following the required operational, fiscal and legal reviews.  Approval at this time will have no impact on DPH’s TB prevention and control activities.
