RECOMMENDATION(S)

Recommendation

Approve Amendment No. 3, effective August 21, 2019, to contracts with the following agencies for the provision of medical care and support under Ryan White Part A, increasing the total amount by $7,167,775, from $19,635,039 to $26,802,814, and exercising the first one-year option to extend, for the total contract period of March 1, 2017 through February 28, 2021:

1.                     AIDS Healthcare Foundation, Contract No. 17-79, increasing by $389,861, from $1,015,309 to $1,405,170.

2.                     County of Riverside, Department of Public Health, Contract No. 17-80, increasing by $785,468, from $2,382,707 to $3,168,175.

3.                     Desert AIDS Project, Contract No. 17-81, increasing by $2,721,222, from $7,824,022 to $10,545,244.

4.                     Foothill AIDS Project, Contract No. 17-82, increasing by $2,723,308, from $6,897,079 to $9,620,387.

5.                     Social Action Community Health Systems, Contract No. 17-84, increasing by $547,916, from $1,515,922 to $2,063,838.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost).  The amendments reflect an overall increase of $7,167,775 to the total for these services, which realigns with the full award amount received by the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) and reflects the allocation and funding priorities made by the Inland Empire HIV Planning Council (IEHPC) for grant year 2019-20.  Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2019-20 budget and will be included in future recommended budgets.

 

BACKGROUND INFORMATION

Under the terms of the recommended amendments, DPH will continue to provide comprehensive medical care and support services through contracted vendors to approximately 3,300 persons living with HIV/AIDS in San Bernardino and Riverside counties.  Services include medical care, mental health services, dental care, substance abuse treatment, early intervention services, transportation, housing, and food assistance. 

 

The IEHPC is the planning body appointed by the San Bernardino County Board of Supervisors (Board) and mandated by Federal law to set service priorities for funding allocations for the expenditures of Ryan White Program funds.  

 

The contract amendments reflect the service allocation adjustments made by IEHPC and the increase in overall funding.  Revised budgets and scopes of work were obtained from the contractors for Program Year 2019-20 and are included in the amendments.  

 

Contractor performance will be measured by reviewing monthly invoicing and data reports based on client visits, and annual site visits by County staff to ensure compliance with the administrative, program, and fiscal terms and conditions of the contracts, with special attention given to the Ryan White payer of last resort requirement.

 

PROCUREMENT

As a result of a formal procurement, the Board approved contracts on February 14, 2017 (Item No. 44) to provide medical care and support services for the Ryan White Part A and Minority AIDS Initiative Program, in the amount of $19,500,000 ($6,500,000 annually), for the three-year contract period of March 1, 2017 through February 29, 2020.

 

On October 3, 2017 (Item No. 23), the Board approved Amendment No. 1 to these contracts increasing the total dollar amount by $367,911, from $19,500,000 to $19,867,911. 

 

On December 4, 2018 (Item No. 44), the Board approved Amendment No. 2 to these contracts decreasing the total dollar amount by $232,872, from $19,867,911 to $19,635,039. 

 

The current recommended amendments will increase the total amount of these contracts by $7,167,775 to $26,802,814, for the remaining contract period of March 1, 2019 through February 28, 2021, which reflects an overall increase from the previous 2019-20 funding award and adds funding for 2020-21.  A new procurement for these services will be conducted in 2020.
