Recommendation
Approve contract with Mt. Rubidouxidence Opco LLC, doing business as Jurupa Hills Post Acute Care, to provide Post-Acute Care Skilled Nursing to uninsured patients of Arrowhead Regional Medical Center in an amount not to exceed $250,000 for the period of December 17, 2019 through March 31, 2020.

FINANCIAL IMPACT
Approval of a contract with Mt. Rubidouxidence Opco LLC, doing business as Jurupa Hills Post-Acute Care, (Jurupa Hills) will not result in the use of Discretionary General Funding (Net County Cost) as the $250,000 for the Skilled Nursing Facility is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Sufficient appropriation and revenue are included in the Arrowhead Regional Medical Center (ARMC) 2019-20 budget.
BACKGROUND INFORMATION
Currently, ARMC has uninsured patients that no longer meet inpatient criteria and require a lower level of care. The lower level of care includes rehabilitative services, wound care, IV antibiotic therapy, dietary services, sitters for behavioral health patients and other therapies which have been prescribed as a result of an accident, injury, illness or disability. These uninsured patients do not have the financial resources to pay for their additional post discharge care, therefore a skilled nursing facility is the appropriate next level of care. Approval of this item will provide for the safety, health and social service needs of county residents as there is an opportunity to reduce costs to ARMC, improve quality of care, and increase overall reimbursement for ARMC.
ARMC has transferred uninsured patients who need a lower level of care to Jurupa Hills, a Skilled Nursing Facility (SNF), since 2011. Jurupa Hills was the only SNF willing to take ARMC uninsured patients with ARMC paying the costs. A SNF is utilized when ARMC is near capacity and have patients that require a lower level of care.
Approval of this non-competitive contract will allow ARMC the ability to release lower level of care patients to Jurupa Hills. The current cost to treat a patient at ARMC is $3,200 per day. The cost to ARMC to move a patient to Jurupa Hills is $550 per day per patient. The cost savings is significant but most importantly, it can help alleviate the high demand for beds for those patients who are in need of an inpatient hospital stay.
PROCUREMENT
ARMC published a Request for Proposal (RFP) in April of 2018 through the County of San Bernardino eProcurement System. The RFP was sent to over sixty vendors and only one vendor submitted a response. The proposal was rejected as the proposal was for a senior living facility, where assistance provided is very limited instead of a skilled nursing facility, where assistance is provided is at a higher level. A new RFP has been released and a new contract will be awarded prior to March 31, 2020.
Based on the above, ARMC is recommending this contract with Jurupa Hills until the RFP is completed. Jurupa Hills is considered a non-competitive procurement. In addition, the County recently issued a non-competitive Purchase Order to this vendor for these services in the amount of $139,335 for prior invoices and ARMC expects to spend more than County Policy before the RFP process is completed.
