FINANCIAL IMPACT

Approval of this item will not impact Discretionary General Funding (Net County Cost). The total cost of these pediatric physician services will not exceed $2,698,920 for the contract period and is funded by fee-for-services Medi-Cal, Managed Care Medi-Cal, private insurance, realignment funds, and sliding fee scale clients.  Payment will be on a fee-for-service basis.  Adequate appropriation and revenue will be included in the Department of Public Health’s (DPH) 2020-21 recommended budget and in future recommended budgets.

 

BACKGROUND INFORMATION

Section 330 of the Public Health Services Act requires that Federally Qualified Health Centers (FQHCs) serve an underserved area or population, offer a sliding fee scale, and provide comprehensive health services which includes pediatric services.  Currently, DPH operates four FQHCs in San Bernardino, Ontario, Hesperia and Adelanto.  Comprehensive pediatric services were integrated into the DPH health centers in 2014 and 2015. 

 

Under the terms of the recommended contract with Faculty Physicians and Surgeons of LLUSM dba Loma Linda University Faculty Medical Group (LLUFMG), board certified pediatric physicians will provide comprehensive pediatric services at each FQHC based on patient need and volume.  The schedule will be coordinated with DPH Clinic Operations Section.  In addition, LLUFMG will participate in administrative duties by assisting DPH in developing and implementing policies and procedures, long-range strategic plans, chart activities to meet performance measures, and participate in continuous quality improvement/quality assurance. 

 

Approval of this item will allow DPH to enter into a contract with LLUFMG to obtain licensed physicians to provide the necessary pediatric services at the FQHCs, for the contract period of July 1, 2020 through June 30, 2023, with the option to extend two additional years.

 

PROCUREMENT

In August 2019, the County Administrative Office approved and authorized the release of a Request for Proposals (RFP) to solicit for doctors or physician groups to provide qualified, licensed board certified physicians (pediatricians) to meet the demand of providing comprehensive pediatric services at the FQHCs.

 

The RFP was posted on the County of San Bernardino’s Electronic Procurement Network (ePro) system and mailed to prospective bidders.  Two proposals were received by the established deadline.  However, upon initial review, one of the proposals was deemed non-responsive.  The remaining proposal submitted by LLUFMG was reviewed and found to meet the requirements of the RFP and is able to provide the necessary pediatric physician services.  As a result of contract negotiations, modified insurance language was approved by the County’s Risk Management.  Either party can terminate the Contract without cause by serving a written notice to the other party 60 days in advance of termination.
