Recommendation

Approve Amendment No. 1 to Contract 18-341, effective July 1, 2020, with Pacific Clinics for Adult Mental Health Clubhouse Services, expanding services to Needles and increasing the amount by $281,680, from $2,067,000 to $2,348,680, with no change to the contract period of July 1, 2018 through June 30, 2021.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The increase of $281,680 for Adult Mental Health Clubhouse services will be funded 100% by Mental Health Services Act (MHSA) funding. Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2020-21 budget.

 

BACKGROUND INFORMATION

Adult Mental Health Clubhouses provide an array of mental health service interventions to assist adults 18 and over in their recovery from mental illness to function positively in their work, personal and social environments. Interventions are designed to provide maximum reduction of mental disabilities to restore or maintain enhanced self-sufficiency. The recommended amendment to the contract with Pacific Clinics will allow DBH to expand services to Needles.  DBH anticipates that Pacific Clinics will provide Adult Mental Health Clubhouse services to an estimated 800 additional participants located in the Needles area, at an average cost of $352 per participant.

 

Clubhouses act as a community designed to promote confidence, social skills, and opportunities to access education, housing, health care, and employment.  Services are rooted in the Recovery, Wellness and Resilience approach by collaborating with the members to encourage goals of self-management, hope, empowerment, and reintegration back into their community as a contributing member, with the goal of creating a restorative environment to counteract internal and external stigma.  Members are engaged in the running of the clubhouse while working side-by side with staff in operations and providing feedback.  This model is based on a work-ordered day consisting of organized, structured work activities designed to motivate members in the accomplishment of tasks required to fulfill Clubhouse objectives.  Projected outcomes for each Clubhouse are to reduce suffering associated with serious mental illness by increasing resiliency, self-help skills, and making one’s own choices to achieve a satisfying and fulfilling life.

 

PROCUREMENT

On June 12, 2018 (Item No. 27), as the result of a formal procurement, the Board of Supervisors (Board) approved contracts with Pacific Clinics ($2,067,000) and South Coast Community Services ($900,000) to provide Adult Mental Health Clubhouse services, for the period of July 1, 2018 through June 30, 2021, with two one-year extension options contingent upon funding and contractor performance.  Pacific Clinics provides services to the High Desert, Central Valley, and Morongo Basin Desert regions; South Coast Community Services provides services to the East Valley region. 

 

During the past year, DBH has been made aware of the need for MHSA funded Adult Mental Health Clubhouse services within the Needles area.  All MHSA services are required to be developed, implemented, and evaluated through a community program planning stakeholder process.  The stakeholder process identified Needles as an underserved area of San Bernardino County, providing a low level of highly effective peer support programs.  Subsequently, at a community stakeholder meeting with the DBH Director, a signed petition was presented requesting and outlining a need for an Adult Mental Health Clubhouse in their community.  As this is a remote area of San Bernardino County with limited service vendors, DBH determined that the best option was to solicit the existing providers of Adult Mental Health Clubhouse services in order to add the new service area of Needles to an existing contract.

 

On March 27, 2020, DBH sent Letters of Interest to Pacific Clinics and South Coast Community Services who are currently providing services in other areas of San Bernardino County, to soliciting interest in providing services in the Needles area.  Pacific Clinics was the only agency to agree to provide the services to the requested location.

 

The recommended amendment will allow Pacific Clinics to expand their Adult Mental Health Clubhouse Services to Needles.  DBH will continue to monitor the contract on a regular basis to ensure compliance standards are met.
