Recommendation
1. Acting as the governing body of the County of San Bernardino, approve Amendment No. 1 to Memorandum of Understanding No. 18-404 between Arrowhead Regional Medical Center (ARMC) and Inland Counties Emergency Medical Agency (ICEMA) for designation as a Level II Trauma Center, extending the term by an additional three (3) years, from July 1, 2021 through June 30, 2024, in which ARMC shall continue to pay ICEMA the annual fee of $25,000 for costs associated with designating ARMC as a Level II Trauma Center.
2. Acting as the governing body of the Inland Counties Emergency Medical Agency, approve Amendment No. 1 to Memorandum of Understanding No. 18-404, between Arrowhead Regional Medical Center and Inland Counties Emergency Medical Agency for designation as a Level II Trauma Center, extending the term by an additional three years, from July 1, 2021 through June 30, 2024, in which ARMC shall continue to pay ICEMA the annual fee of $25,000 for costs associated with designating ARMC as a Level II Trauma Center.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Arrowhead Regional Medical Center annual costs in the amount of $25,000 are funded by Medi-Cal, Medicare, private insurances, and other departmental revenue. Sufficient appropriation and revenue has been included in the ICEMA and ARMC 2021-22 recommended budgets and will be included in future recommended budgets.
BACKGROUND INFORMATION
ICEMA is requesting to amend the existing Memorandum of Understanding (MOU) between ARMC and ICEMA designating ARMC as a Level II Trauma Center, by extending the term for an additional three year period, from July 1, 2021 through June 30, 2024, in the amount of $25,000 annually.
California Health and Safety Code, Section 1798.165, provides the authority for a Local Emergency Medical Services Agency (LEMSA) to designate trauma facilities as part of the trauma care system pursuant to State Regulations. Additionally, the California Code of Regulations, Division 9, Chapter 7, Section 100255 requires LEMSA’s and designated trauma centers to enter into written agreements. ARMC is currently designated as a Level II Trauma Center (Adult and Pediatric), meeting the standards developed by ICEMA for designation as a Level II Trauma Center and providing for the safety, health and social services needs of the residents of the County of San Bernardino by expediting transport of trauma victims to Trauma Centers specifically equipped for the treatment of trauma patients.
ICEMA is the LEMSA responsible for implementing the ICEMA Trauma Care System Plan, which was approved by the ICEMA Board of Directors (Board) on December 18, 2001 (Item No. 108). The Trauma Care System Plan provides for a formal basis for implementing, evaluating and improving the trauma system in the ICEMA region.
On July 15, 2008 (Item No. 46), the Board approved Agreement No. 08-735 with ARMC to provide trauma services in the ICEMA region, for the period July 1, 2008 through December 31, 2010, with two additional two-year periods.
On June 14, 2011 (Item No. 72), the Board approved Amendment No. 1 to Contract No. 08-735 with ARMC to continue to provide trauma services as a designated Level II Trauma Center within the ICEMA region. Amendment No. 1 extended the agreement for the period of January 1, 2011 through December 31, 2012, with the option to renew the agreement for two additional two-year periods.
On May 6, 2014 (Item No. 76), the Board approved MOU No. 14-302 with ARMC to continue to provide trauma services as a designated Level II Trauma Center within the ICEMA region, for the period of May 6, 2014 through June 30, 2015, with an option to renew for an additional three-year period. On July 15, 2015, via letter, ICEMA exercised the option to extend the contract for an additional three-year period until June 30, 2018.
On June 26, 2018 (Item No. 86), the Board approved MOU No. 18-404 with ARMC to provide trauma services in the ICEMA region, for the period July 1, 2018 through June 30, 2021, with the option to extend for an additional term of three years on the same terms and conditions set forth in the MOU, by written approval of ARMC and ICEMA’s Board of Directors.
Approval of this item will allow ICEMA to continue the MOU with ARMC to provide trauma services within the ICEMA region, for the period of July 1, 2021 through June 30, 2024.
ICEMA, as the local Emergency Medical Services (EMS) agency, is responsible for ensuring effective EMS for the Counties of San Bernardino, Inyo, and Mono. ICEMA administers State and local codes and regulations governing the provision of emergency medical care to the sick and injured at the scene of an emergency and during transport to a care hospital.
