Recommendation
Approve Amendment No.1, effective July 27, 2021, to the following contracts to provide medical care and support services under the Ending the HIV Epidemic: A Plan for America Program, increasing the contract amount by $5,101,200, from $1,800,000 to $6,901,200, exercising options to extend an additional three years, for the total contract period of October 28, 2020 through February 28, 2025:
1. AIDS Healthcare Foundation, Contract No. 20-1065, increasing the contract by $322,794, from $89,145 to $411,939.
2. Borrego Community Health Foundation, Contract No. 20-1066, increasing the contract by $1,366,658, from $527,935 to $1,894,593.
3. Desert AIDS Project, Contract No. 20-1067, increasing the contract by $1,417,000, from $500,000 to $1,917,000.
4. Foothill AIDS Project, Contract No. 20-1068, increasing the contract by $1,142,974, from $304,323 to $ 1,447,297.
5. Young Scholars for Academic Empowerment dba TruEvolution, Inc., Contract No. 20-1069, increasing the contract by $851,774, from $378,597 to $1,230,371.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The increase of $5,101,200 for HIV medical care and support services is funded by the United States Department of Health and Human Services, Health Resources and Services (HRSA) under the Ending the HIV Epidemic: A Plan for America grant funding. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2021-22 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
The Ending the Epidemic: A Plan for America funding seeks to achieve the important goal of reducing new HIV infections in the United States to less than 3,000 per year by 2030. Across the United States, the initiative promotes and implements four Pillars to substantially reduce HIV transmissions: Diagnose, Treat, Prevent, and Respond. This funding for the combined counties of San Bernardino and Riverside focuses on implementing activities in Pillar Two (Treat) and Pillar Four (Respond). Under the terms of the existing contracts, the agencies in the Recommendation are required to engage in activities that support Pillar Two (Treat) and Pillar Four (Respond) to expand access to HIV care and treatment for people with HIV, both those who are newly diagnosed and those who are not engaged in care, and/or not virally suppressed; and addressing unmet needs and improving client-level health outcomes. In addition, agencies are required to address and respond quickly to HIV cluster detection efforts for those needing HIV care and treatment.
Approval of this item will increase the contract amount and exercise all options to extend the contracts three additional years for continued HIV medical care and support services through February 28, 2025. DPH is requesting exercising the options at this time in order to align with the Ending the HIV Epidemic: A Plan for America’s five-year grant period through February 28, 2025.
Contractor performance will be measured by review of monthly reporting, review of required data submission, and an annual site visit by County staff to monitor compliance with the administrative, program, and fiscal terms and conditions of the contract, with special attention to the Ending the Epidemic: A Plan for America payer of last resort requirement.
PROCUREMENT
On October 27, 2020 (Item No. 65), as a result of a formal procurement, the Board of Supervisors approved contracts with the agencies listed in the Recommendation, in the amount of $1,800,000, for the period of October 28, 2020 through February 28, 2022, with the option to extend up to three one-year periods by mutual agreement of all parties.
