Recommendation
Approve Amendment No.1, effective July 27, 2021 to the following contracts to provide medical care and support services under the Ryan White HIV/AIDS Part A HIV Emergency Relief Grant Program, decreasing the total contract amount by $750,948, from $21,487,377 to $20,736,429, for the contract period of March 1, 2021 through February 29, 2024:
1. AIDS Healthcare Foundation, Contract No. 20-1178, decreasing the contract by $43,335, from $1,244,826 to $1,201,491.
2. Borrego Community Health Foundation, Contract No. 20-1179, decreasing the contract by $52,644, from $1,512,273 to $1,459,629.
3. County of Riverside, Department of Public Health, Contract No. 20-1180, decreasing the contract by $78,633, from $2,245,365 to $2,166,732.
4. Desert AIDS Project, Contract No. 20-1181, decreasing the contract by $276,951, from $7,932,984 to $7,656,033.
5. Foothill AIDS Project, Contract No. 20-1182, decreasing the contract by $228,039, from $6,518,220 to $6,290,181.
6. SAC Health System, Contract No. 20-1183, decreasing the contract by $36,057, from $1,035,768 to $999,711.
7. Young Scholars for Academic Empowerment dba TruEvolution, Inc., Contract No. 20-1184, decreasing the contract by $35,289, from $997,941 to $962,652.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The decrease of $750,948 decreases the total amount of funding for these contracts to $20,736,429 due to a reduction in the amount by the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) allocated for this program. These services are funded by Ryan White HIV/AIDS Part A HIV Emergency Relief Grant (Ryan White Part A) Program ($6,424,886 annually) and the Minority AIDS Initiative (MAI) ($487,256 annually) and will be fully reimbursed by HRSA. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2021-22 budgets and will be included in future recommended budgets.
BACKGROUND INFORMATION
DPH has been receiving funding for the Ryan White Part A Program since 1994. Acting as the lead agency for the Transitional Grant Area of San Bernardino and Riverside counties, DPH oversees and administers the grant funds and assures compliance with grant requirements. Funding for the Ryan White Part A Program provides comprehensive continuum of high quality, community-based medical care and support services for low-income individuals with Human Immunodeficiency Virus (HIV) and their families. MAI funding is used specifically to assist African American and Hispanic individuals with HIV with linkage and retention in care. The Inland Empire HIV Planning Council (IEHPC) is the planning body appointed by the Board of Supervisors and mandated by Federal law to set service priorities for funding allocations for the Ryan White Program. Services include medical care, mental health services, dental care, substance abuse treatment, medical nutrition therapy, early intervention services, case management, transportation, psychosocial support, emergency financial assistance, housing, and food assistance.
Approval of this item decreases the amount of funding to the contractors due to a reduction in funding by HRSA allocated for this program. Contractors were notified of the change and made adjustments to their program budgets and scope of works while adhering to the IEHPC set service priorities. The recommended amendments reflect the changes and the reduction in funding. There is little to no impact on service delivery or program activities.
Contractor performance will be measured by review of monthly reporting, review of required data submission, and an annual site visit by County staff to monitor compliance with the administrative, program, and fiscal terms and conditions of the contract, with special attention to the Ryan White payer of last resort requirement.
PROCUREMENT
On December 15, 2020 (Item No. 55), as a result of a formal procurement, the Board of Supervisors approved contracts with the agencies listed in the Recommendation, in the amount of $21,487,377, for the period of March 1, 2021 through February 29, 2024.
