Recommendation
Ratify the purchase of an unbudgeted fixed asset in the amount of $110,000, for an Optomap Ultra-Widefield Imaging Device, to facilitate early protection from vision impairment or blindness and early detection of life-threatening diseases like diabetes, stroke, and cardiovascular disease.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Due to a delay in opening the Arrowhead Regional Medical Center (ARMC) Fontana Clinic, the Selenia 3D 6000 Package Mammography Machine (Selenia Mammography Machine) in the amount of $400,698 that was approved in the 2020-21 Capital Improvement equipment budget was not purchased. The cost of $110,000 for the Optomap Ultra-Widefield Imaging Device was substituted with the Selenia Mammography Machine. Therefore, there is no additional cost associated with this purchase as sufficient appropriation was included in the ARMC 2020-21 budget.
BACKGROUND INFORMATION
In May 2021, emergency Purchase Order No. 4100199085 was issued to replace the Optomap Ultra-Widefield Imaging Device (Optos P200DTx) in ARMC’s Ophthalmology Clinic. This equipment is used for management of serious eye disease; detection of life-threatening diseases like diabetes, stroke, and cardiovascular disease; and routine eye exams and evaluation. The equipment that was replaced was unrepairable as it had reached end of life and was no longer supported under warranty. Immediate replacement was necessary for continuity of services at the clinic.
ARMC recommends that the Board of Supervisors (Board) ratify this purchase to ensure ARMC continues to provide for the health and safety needs of County residents seeking eye treatment services through its Ophthalmology Clinic.
PROCUREMENT
Purchasing has approved this non-competitive purchase based on the sole source nature of the equipment as it is the only Food and Drug Administration approved product that can produce digital images of up to 82% of the retina in a single, non-contact image capture. San Bernardino County Policy 11-04 Section 8B allows for the emergency purchase of goods, supplies, equipment and services for the preservation of life or property. Purchase of the Optos P200DTx was made in accordance with this policy. This policy also requires a Board agenda item requesting approval and confirmation of emergency purchases for fixed assets necessary for the preservation of life or property that exceed $10,000 per unit.
