RECOMMENDATION(S)

Recommendation

1.                     Approve revised non-financial Master Mobile Medical Clinic Site Agreement between Arrowhead Regional Medical Center and various facilities to use their site for the purpose of providing mobile medical care services to County residents with a term beginning on the date of execution through September 13, 2026, with one two-year option to extend each individual agreement.

2.                     Authorize the Director of Arrowhead Regional Medical Center or Hospital Administrator to execute the individual Site Agreements with facilities and extension options on behalf of the County, subject to County Counsel review.

FINANCIAL IMPACT

Approval of these recommendations will not result in the use of Discretionary General Funding (Net County Cost). The costs for the provision of medical care services provided by the Arrowhead Regional Medical Center (ARMC) Mobile Medical Clinic Program will be funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Sufficient appropriation and revenue are included in the ARMC 2021-22 budget and will be included in future recommended budgets. 

 

BACKGROUND INFORMATION

Establishing Mobile Medical Clinic Site Agreements with various facilities allows ARMC to use their sites to provide mobile primary care services to San Bernardino County residents. ARMC currently partners with several agencies, including the Redlands Family Service Associates, Muscoy Elementary School, the Yucaipa Center for Health and Family Well Being, and the Ruben Campos Community Center, to provide vital health care services to the community. Site Agreements will be executed with new organizations and renewed with existing partners as needed.

 

The Mobile Medical Clinic Program (Program) has been in place since 2008 and assists ARMC in its efforts to improve the health of County residents by providing necessary primary care medical visits.  The Program enables ARMC medical personnel to go to County residents to meet their primary healthcare needs. It also allows the hospital to reach residents who for whatever reasons may not have the means to travel to the ARMC hospital facility or one of ARMC’s Family Health Clinics.

 

The Mobile Medical Clinic Program targets the following under-insured/uninsured populations: the elderly, homeless, public housing residents, low-income school children/adolescents and their families.  The Program will provide these individuals with primary care services while assisting them in obtaining medical coverage and placement in a “Medical Home.”  A Medical Home is a team based health care delivery model led by a physician that provides comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes. 

 

All facilities under the proposed Master Mobile Medical Clinic Site Agreement will be required to provide necessary provisions, including but not limited to, utilities such as phone lines and electrical hookups, as required by the Mobile Medical Clinic.  Under the proposed Master Mobile Medical Clinic Site Agreement, either party may terminate the agreement for any reason with 30 days advance written notice.  Although the proposed Master Mobile Medical Clinic Site Agreement does not contain the standard County insurance provisions generally applicable to vendor contracts, it contains language that the sites will maintain general liability, automobile liability, and workers’ compensation liability insurance with limits adequate to protect against liabilities that may arise out of the Agreement. 

 

On November 4, 2008 (Item No. 52), the Board of Supervisors (Board) accepted the donation of a Mobile Medical Clinic from the ARMC Foundation, enabling ARMC to provide mobile primary care health services to San Bernardino communities.

 

On January 12, 2016 (Item No. 10), the Board approved a Non-Financial Master Mobile Medical Clinic Site Agreement between ARMC and various facilities for the utilization of their site to provide primary care services to San Bernardino County residents for the period of March 27, 2016, through March 26, 2019, with one two-year option to extend.  The Board also authorized the Director of ARMC or Hospital Administrator to execute the individual Site Agreements and the extension options on behalf of the County.

 

On May 24, 2016 (Item No. 20), the Board approved a revision to the Non-Financial Master Mobile Medical Clinic Site Agreement to remove the $25 fee for uninsured patients to eliminate the financial burden on patients and improve access to care.

 

On August 21, 2018 (Item No. 7), the Board approved a revision to the Non-Financial Master Mobile Medical Clinic Site Agreement to establish a new three-year term of August 21, 2018 through August 20, 2021, with one two-year option to extend each individual agreement. The Board also authorized the Director of ARMC or Hospital Administrator to execute the individual Site Agreements and the extension.
