Approve Amendment No. 1, effective October 5, 2021, to Contract No. 19-384 with Generations Healthcare of Lakeside, LLC for the provision of Skilled Nursing Facility neurobehavioral services, decreasing the total contract amount by $330,000 from $1,590,306 to $1,260,306, with no change to the contract period of July 1, 2019 through March 31, 2024.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The decrease of $330,000 to the contract with Generations Healthcare of Lakeside, LLC (Lakeside) for Skilled Nursing Facility (SNF) neurobehavioral services will be reallocated within the programs supported by the Community Mental Health Services Block Grant funds.  Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2021-22 budget and will be included in future recommended budgets.

 

BACKGROUND INFORMATION

Lakeside provides SNF neurobehavioral services to severely persistently mentally ill (SPMI) consumers, whose needs require complete inpatient psychiatric and medical care. SNFs are licensed by the State of California Department of Social Services and Department of Health Care Services. Placement of consumers in SNFs offers an appropriate level of care and is less costly than placement within State Hospitals. Due to the severity of mental and/or medical conditions experienced by DBH’s hardest to serve clients, other types of facilities are not willing to accept them. Since 2012, Lakeside has successfully provided neurobehavioral services to DBH clients, accepting ambulatory clients that are older with challenging psychiatric and medical conditions who are being discharged from a long term locked facility, such as Patton or Metropolitan State Hospital.

 

During a recent review, DBH was made aware that Lakeside is not maximizing the utilization of Contract No. 19-384. Therefore, DBH is recommending the contract with Lakeside be amended to decrease the total contract amount by $330,000, as well as decreasing the number of approved beds from eight to four. This reduction will allow DBH to reallocate valuable funding resources to other programs in need of and supported by the Community Mental Health Services Block Grant funds.    

 

PROCUREMENT

On June 11, 2019 (Item No. 18), the Board of Supervisors approved the non-competitive Contract No. 19-384 with Lakeside for SNF neurobehavioral services, in the amount of $1,590,306, for the period of July 1, 2019 through March 31, 2024. DBH routinely searches and remains open to SNF facilities that are qualified neurobehavioral service providers and willing to accept consumers with psychiatric needs and severe medical issues. Purchasing supports the non-competitive procurement for this service with a specialized credentials justification.

 

Contractor performance will continue to be measured by reviewing required monthly reports and annual site visits by DBH staff to ensure compliance standards are met.
