RECOMMENDATION(S)

Recommendation

Approve Amendment No. 2, effective November 16, 2021, to the following contracts to provide medical care and support services under the Ryan White HIV/AIDS Part A HIV Emergency Relief Grant Program, adjusting the Budget and Scope of Work, with no change to the total contract amount of $2,459,340, for the contract period of March 1, 2021 through February 29, 2024: 

1.                     Borrego Community Health Foundation, Contract No. 20-1179, decreasing the amount by $100,000, from $1,459,629 to $1,359,629.

2.                     SAC Health System, Contract No. 20-1183, increasing the amount by $100,000, from $999,711 to $1,099,711.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost).  The total combined amount of $2,459,340 for the medical care and support services provided by the contracts in the Recommendations remains the same as the $100,000 will be reallocated from Borrego Community Health Foundation’s (Borrego) contract to SAC Health System’s (SACHS) contract in order to optimize program funding.  The Ryan White HIV/AIDS Part A HIV Emergency Relief Grant Program (Ryan White Part A Program) is funded by the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA).  Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2021-22 budget and will be included in future recommended budgets.

 

BACKGROUND INFORMATION

DPH has been receiving funding for the Ryan White Part A Program since 1994.  Acting as the lead agency for the Transitional Grant Area of San Bernardino and Riverside counties, DPH oversees and administers the grant funds and assures compliance with grant requirements. Funding for the Ryan White Part A Program provides comprehensive continuum of high quality, community-based medical care and support services for low-income individuals with Human Immunodeficiency Virus (HIV) and their families.  Ryan White Part A funding also includes Minority Aids Initiative (MAI) funding, which is used specifically to assist African American and Hispanic individuals with HIV with linkage and retention in care.  The Inland Empire HIV Planning Council (IEHPC) is the planning body appointed by the Board of Supervisors and mandated by Federal law to set service priorities for funding allocations for the Ryan White Program.  Services include medical care, mental health services, oral health/dental care, substance abuse treatment, medical nutrition therapy, early intervention services, case management, transportation, psychosocial support, emergency financial assistance, housing, and food assistance.

 

The recommended amendments will adjust the Scope of Work and Budget for the contracts with Borrego and SACHS.  Borrego and SACHS provide oral healthcare/dental services through their respective contracts in the Recommendations.  DPH was recently informed that Borrego is not able to spend the full amount allocated to their contract for these services.  In order to optimize Ryan White A Program funding, DPH is requesting that $100,000 be reallocated to the contract with SACHS as this agency has an increased need and the capacity to provide the necessary Ryan White Part A program oral healthcare/dental services.  Approval of this item will allow DPH to maintain the same level of service for this service category and maintain compliance with the priorities set by IEHPC.

 

Contractor performance will be measured by review of monthly reporting, review of required data submission, and an annual site visit by County staff to monitor compliance with the administrative, program, and fiscal terms and conditions of the contract, with special attention to the Ryan White payer of last resort requirement.

 

PROCUREMENT

On December 15, 2020 (Item No. 55), as a result of a formal procurement, the Board of Supervisors (Board) approved Contract No. 20-1179 with Borrego in the amount $1,512,273, and Contract No. 20-1183 with SACHS in the amount of $1,035,768, to provide medical care and support services under the Ryan White Part A Program, for the period of March 1, 2021 through February 29, 2024.

 

On July 27, 2021 (Item No. 36), the Board approved Amendment No. 1, effective July 27, 2021, decreasing Borrego’s contract amount by $52,644, from $1,512,273 to $1,459,629, and SACHS’ contract by $36,057, from $1,035,768 to $999,711, due to an overall reduction in funding by HRSA, with no change to the contract period.
