RECOMMENDATION(S)
Recommendation
1. Accept and approve agreement (State Agreement No. 202236) for the following grants from the California Department of Public Health for the period of July 1, 2022 through June 30, 2023, for the following programs:
a. Maternal, Child, and Adolescent Health Program, in the amount of $541,781.
b. Black Infant Health Program, in the amount of $1,461,127.
c. Black Infant Health Program expansion funding, in the amount of $1,195,350.
2. Designate the Chair of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Public Health to execute and submit any subsequent non-substantive amendments to the grant awards on behalf of the County, subject to review by County Counsel.
3. Direct the Chair of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Public Health to transmit all non-substantive amendments in relation to the grant awards to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The grant funding from the California Department of Public Health (CDPH) for the Maternal, Child, and Adolescent Health (MCAH) Program, in the amount of $541,781, and the Black Infant Health (BIH) Program, in the amount of $2,656,477, consists of federal Title V, Sudden Infant Death Syndrome Program (part of Title V), State General Fund, and/or federal Title XIX funds. The MCAH Program includes a local match of $371,309, and the BIH Program includes a local match of $220,319, which will both be met with 1991 Realignment funds. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2022-23 budget.

BACKGROUND INFORMATION
CDPH provides non-competitive funding for the MCAH and BIH Programs to local health departments to promote healthy living for mothers and their families through a focus on the comprehensive health, physical, and psychosocial needs of the maternal, child, and adolescent health population. DPH has received funding for the MCAH Program since 1987, and the BIH Program from 1990 through 2010 and again beginning in 2014. In 2022-23, DPH anticipates the MCAH Program will provide services to approximately 26,144 pregnant and parenting women and their children and families countywide at an estimated cost of $21 per individual; and the BIH Program will provide services to up to 371 African-American women countywide, at an estimated annual cost of $7,160 per individual.
Through the MCAH Program, DPH performs population-based, core public health functions, including assessment, policy development, and health improvement activities. Program staff collaborate with healthcare providers, community-based agencies, and community groups and members to identify health issues that affect women, infants, and children and develop best practices and interventions that will increase and expand access to care for mothers and children, especially those with low income or limited access to services. Further, the MCAH Program promotes the need for appropriate levels of care to improve pregnancy outcomes and reduce maternal and infant morbidity and mortality.
The BIH Program is designed to meet the health-related needs of pregnant and postpartum Black and African-American women through a State evidence-informed model that features group intervention to encourage empowerment and social support in the context of a life course perspective and through social service case management to link clients with the needed community and health related services.
On September 27, 2022 (Item No. 43), the Board of Supervisors (Board) approved the submission of the combined agreement funding application for the MCAH ($558,206) and BIH ($1,461,127) programs, for the period of July 1, 2022 through June 30, 2023. On November 9, 2022, DPH received approval for the MCAH agreement funding application; the approval for the BIH agreement funding application was received by DPH on November 17, 2022. Acceptance of the grant awards constitutes a binding agreement, and a subsequent agreement will not be expected. The final budget documents for both programs are for the period of July 1, 2022 through June 30, 2023.
On November 17, 2022, DPH received notification from CDPH of the intent to award BIH funding for administration and expansion of BIH, aligning with budget language that amends the CDPH allocation for local health jurisdictions that currently receive State General Fund allocations for the BIH program. This Notice of Intent to Award increased the BIH funding available for the period of July 1, 2022 through June 30, 2023 in the amount of $1,195,350.
This item is being presented at this time, as this is the first available date following the negotiations with the state regarding the revised program budgets and other funding documentation, as well as required operational, fiscal, and legal reviews. Approval by the Board at this time will not impact DPH’s MCAH and BIH services or activities.
